Our office is open Monday through Friday from 7:30 a.m. to 4:30 p.m.  To make an appointment, please call our office at (269) 969-6251 or complete the online appointment form.

• Emergencies
  
  If you call our office during non-business hours, you will be able to reach us through our answering service.  The service will contact our physician on call as needed.  If your emergency is severe, you should go immediately to the emergency room.  The emergency room doctor can notify the physician on call for our practice.

• Your First Visit
  
  Please arrive 15 minutes before your scheduled appointment time to complete the necessary forms.  If you received a pre-registration packet in the mail please bring it with you if you didn’t return it prior to your appointment.  Please bring a current medication list with you indicating the name of the medication(s) you are currently taking and the dosage.
  
  Please bring all of your insurance cards with you to your appointment and a valid driver’s license or state identification card. 
  
  If you have a managed care insurance plan, please be sure you have received authorization for your appointment before you come to our office.
  
  If your injury is work related we will require an authorization, claim number and billing information from your employer. 
  
  If your injury is related to a motor vehicle accident or injury, you will need to provide us with the claim number and billing information when you check in for your appointment.

• Contacting Our Office
  
  If you have a question regarding your medical care or a prescription, please call our office between 7:30 am and 4:30 pm at (269) 969-6251.  You will be directed to our phone nurse or the medication refill voice mail.  If your question requires the attention of your physician, the nurse will consult with him as soon as possible and return your call by the end of the business day.
  
  If  you have a question regarding how your insurance company paid on your account, please contact your insurance company directly before calling us.  If you continue to have questions regarding your account please contact our billing office at (269) 969-6052 between the hours of 7:30 am and 4:30 pm.

• Insurance
  
  We participate with many insurance plans.  While we will try to assist you in any way we can, it is ultimately the patient’s responsibility to pay for all services received.  Please be sure to have all authorizations and referrals in place prior to your visit with us.  If you need additional help trying to navigate the complexities of your insurance coverage, please feel free to contact our billing department at (269) 969-6052 and they will try to assist you with your questions.
